One2One Home Care at Lincoln Home Says Thank You to Veterans for Their Service

To all veterans of all branches: Thank you for your sacrifice, your bravery, and the example you set for us all. In short, thank you for your service! 
 
The VA provides health care for Veterans from providers such as One2One Home Care in local communities. Veterans may be eligible to receive care from a local community provider when the VA cannot provide the care needed. This care is provided on behalf of and paid for by VA. 
 
Once a Veteran makes out the application and is found to be qualified a referral goes out to the area Home Care providers. Community care is available to Veterans based on certain conditions and eligibility requirements, and in consideration of a Veteran’s specific needs and circumstances. Community care must be first authorized by the VA before a Veteran can receive care from a community provider. 
A Veteran may also request a certain provider if they have a preference. The provider then schedules a home visit with the Veteran. This meeting includes setting up weekly services to be provided by a Caregiver. 
 
Veterans are encouraged to call the VA to request assistance with Transportation, Home Services, Home Health, Medical Assistance, VA Physician Contact, Appointments for other Medical Services, Social Worker Services, Pension and Disability Payments.
